    You go to the Aging/Disability office for your county. They will help
    you with applying for aid or direct you to the office that will handle
    that for you. Social Services for the county will most likely be the
    follow up they’ll direct you to. But they will refer companies that do
    some but not all, aiding a person who has the inability to clean their
    homes. Or shower and dress themselves. Or also be able to cook
    their own meals, or even walk without assistance. They are not
    allowed to do anything of an invasive task. Such as cutting toenails,
    or hygiene. They are there to assist, if they can. But from my exper-
    ience their help is centered on house cleaning and doing laundry in
    the home. As to whether these businesses are licensed, I can’t tell
    you. I know that their representatives are not bonded. So that can be
    an issue. And also how well they know how to perform certain house-
    hold tasks can be another, if they have little work experience from
    being new to the job market because they are too young. You will
    have to research long before you hire. If you are wanting to do such
    work, rather than hiring or getting free help in the home, you will have
    to speak to a very knowledgeable person who works for the Aging
    Disabilities’ department. They are the authorities to see.

    Your looking for a form, to submit to someone, for what reason?
    Authorization to recieve a beneift?
    Authorization as in regarding HIPAA and being able to discuss a persons personal information?
    Prior authorization, as in, you want to fill it out, submit it, and hope that noone catches that it is not real?
    Any organization that needs one of these own, provides the forms to the public, if there is a need for the public to fill one out on thier own.
    There are many rules by Medicare, Medicaid, private insurance companies, that one must follow to get the benefits paid for, for in home health care, and most, start with a physician making the order for need of care.

    I dont think this is something you can do on your own. Every funding agency or insurance company is going to have their own form. Try Area Agency on Aging in your community.

    They are a reputable company that works with the families to give in home care. Though they are not free. Maybe the state can give you in home care through Medicade.
